    Hi,
    I am on a k-1 visa and about to apply for adjustment of status. I wanted to know if it's mandatory to provide the vaccination records when applying for the adjustment of status (I-485), considering that the records were in the sealed envelope which I had to turn in at the port of entry, and gave them to the customs officer. As you can see, I don't have these records or copy of them. This is basically my question. Thank you for your quick response

      The vaccination supplement of I-693 is required as a supporting document to I-485. It seems like some people have been able to adjust status using DS-3025 of the medical exam that was done before the visa interview. Apparently, that would only work if the DS-3025 was “properly completed”, as they call it. If not, the vaccination supplement of I-693 is required. If you have a copy of DS-3025 and it is complete, it might work to submit it instead of I-693, but there’s always a risk that it leads to an RFE. Otherwise I don’t see any other option than going to a civil surgeon to obtain the vaccination supplement of I-693.
