Mafzy,
You need to add forms I-765 and I-131, an immigrant medical exam (form I-693) in sealed envelope, checks for $420 and $1,070.
You need to include copies of birth certs, marriage cert, I-94, sponsor's 2013 Federal tax return, W2 copies and employment verification.
Include two passport-style photos of you with I-485, G-325A,I-765 and I-131, and one photo with I-130.
Wife should include two photos with her G-325A (attached to I-485) and one passport-style photos with I-130.
I usually submit as separate packets in same mailing the I-485, I-864, I-765, I-131 and I-130. Each packet has its own set of supporting documents and appropriate passport-style photos.

Best not to use separate envelopes with same mailing. Just put separate packets in individual clear plastic inserts, with form G-1145 on top of primary form (I-485) and cover letter below that listing all contents. Attach both checks (in clear plastic protector) on cover letter.
Proof of relationship and commingling of business affairs is not needed until your interview.

Your I-864 should be accompanied by copy of your 2013 Federal income tax return, W2 or 1099 forms, employment verification letter.

The primary form of the Federal income tax return (2013) is Form 1040. You can download your most recent I-94.

I always send G-325A forms from petitioner and beneficiary, with both I-485 and I-130.
The stipulated requirement, however, is only beneficiary G-325A with I-485, but a G-325A from both parties with the I-130.

You need supporting documents with the I-485 and I-130 packets, each of which must stand by itself. Included among your supporting documents should be passport-style photos of each, marriage, birth and any divorce documents. Your I-864 package should include copy of the sponsor's 2013 Federal tax return, W2 copies, and an employment verification letter. Unless you are using cash in bank in lieu of employment income, bank statements are not needed.
