Form I-485 Family Name (Last Name)

  • Form I-485 Family Name (Last Name)

    Seems so simple of a question, but the devil is in the details with these forms. My wife took my last name in our wedding, but what do we put in the Family Name section now? Do we put her maiden name or the new last name? All of the other documents such as passport/uscis case stuff all refer to her maiden name. Thus, my confusion as what to put.

    The other question not so sure about is current uscis status. the status on the uscis site is "post decision activity" so that is what I put.

    If you want your wife's Green Card to have her married name (which is how it should be), use your family name for her family name on form I-485.

    You should also use her married name on the accompanying G-325A, I-864, I-765 and I-131.

    Status is "K1 pending Adjustment," or substitute whichever visa type she entered with for "K1."

      What about for I 130? What do you put there? Maiden or married name?

        The same. Her new primary name should be her married name, unless she prefers to use a prior name for business or professional reasons.
