Not enough lines for addresses on G-325a

The biographical information form asks for addresses within the last 5 years; however, my wife has lived in many different places within the last 5 years and there are not enough lines for her. Usually the forms say "if you need more space, see instructions/add attachment" this section doesn't say anything about adding an attachment. Do I need to add an attachment for the addresses? If I do, what format should I do the attachment page in?

On last line of address area put "See attachment."

On attachment, print "Attachment to G-325A, name, form number, page and section number."
