We are in the process of applying for adjustment of status based on marriage to a USC what are the supporting documents/ evidence to be submitted along to USCIS before our interview? Also which affidavit of support form should my sponsor file?

The forms you need to submit are I-130, I-485, G-325A for both, I-765, I-131, I-864. Fees are $420 and $1,070
Supporting docs are birth, marriage divorce, I-94, visa copy, 2013 tax return, W2's, employment verification.

Can my wife who is the petitioner be my only sponsor? She earns $40 per hour works for 12 hours and 4/3 times a week!

If your wife's documented income annualizes to equal or exceed the required income amount: $19,662, and she has an income tax return for the most recent taxable year, she should meet and have what is needed to sponsor you.
