    Hi
    My wife and I are about to submit our green card package and I wanted to make sure I’m not missing any forms or supporting documents. My wife is American by birth and I am Canadian and we are living in the US. We have no children or past marriages, no criminal history or anything like that. I am legally in the US.
    This is what I have. Please let me know if there is anything else i will need.

    I-130
    I-485
    I-865
    I-765
    G-325a (1 each)
    G-1145

    Supporting documents:
    Medical exam (sealed)
    Wife’s 1040’s past 3 years
    Wife’s current letter of employment
    6 passport photos for my, 2 for my wife
    Copy of birth certificates
    Copy of passport and visa stamp
    Most recent I-94 info
    Copy of wedding certificate
    Album of photos together
    Honeymoon info
    printout of flights together and visiting
    Copies of wedding cards from family

    There should be an I-864 from your wife, rather than an I-865. Also, include an I-131 for an Advance Parole, since your existing visa may be barred from use if you return to Canada and are returning to the U.S.

    Include wife's W2 forms for most recent tax year (2013).

    Fee checks, separate, to "U.S. Department of Homeland Security," are $420 and $1,070.

      Thank you! The 865 was a typo, it is the 864 i have. I was reading somewhere that there are 864a, 864ez and a few others. do these need to be filled out as well?

      As for the I-131, in Part2 i have selected 1d. Now, if i dont have a date for travel, I have been told to write "none" and check the "No" box in question 3a and then skip to section 7. Is this correct?

        For the I-131, Part 2, 1.d. is correct.

        I usually put a start date of travel at about 3 or 4 months from when the package is submitted, just to provide justification.

        Check "no" for 3.a. and 4.a.
