Please help i have a question i-94

Hi Rayb,

thanks for your help so far,i really appreciate it.i have a question,concerning I-485 adjustment status form for my step daughter,

the ( Do you have an Arrival/Departure Record (I-94) Number? ) Should i click NO because being only 10 years she used the same I-94 with her mom,and if i should say YES do i put in my wife's information as

( Name exactly as it appears on Form I-94 that is what is written on the I-94 card,) they have only one I-94 which they both used and which has only my wife's name on it,so should my wife use her name as info on the I-94 or should she just say NO I-94 for our daughter.

Have you tried to download an I-94 for the daughter, from: http://tinyurl.com/n5g376c

If there is no I-94 available at that site for the daughter, is there an entry annotation in the daughter's passport?

I have tried to download but there is none for daughter,only wife and yes she has a stamp on her passport showing the same date with my wife on her point of entry here .

Then use her entry stamp from the passport as proof of entry. Using the mom's I-94 number is not what I would do, i.e., "no I-94 for the daughter."

Hi rayb,

Please another question for you again,does daughter have to do the I-693 medical exam for adjustment of status or is it just my wife alone.she is 10 years old

All applicants, including daughter, must have the I-693 medical exam record in sealed envelope.
