    The applicant - mother, does not speak or understand English, all questions were translated and read to her in her native language
    The from was physically filled out by her son (he is the one who translated and read questions to her) in mother's presents based on answers received

    In this situation, should the Part6 (Signature of Person Preparing the Form, If Other Than Above) be filled by the son? If the son is not a firm, what should be put in the box "Firm Name and Address"?

    I had a similar situation because I filled in forms for my 6 year old son. I completed the sections on "Signature of Person Preparing Form, If Other Than Above"...here I filled in my name and address and wrote my relationship as mother. I also signed my signature in all the areas he was required to sign.

    My forms were accepted. Our situations may be a little different based on the age range of the applicants, but I hope this helps.
