I-94 confusion (is this the form I need?)

Couple of questions, most important one first...

1) I-94... am I looking at the correct form? The copy I have was printed from the cbp.gov website after entering my Fiancee's information. On the "Immihelp guide for adjustment of status" on this website, the instructions state to include the I-94, both sides. I do not have 2 sides; i have a relatively small piece of paper with the I-94 #, arrival date, class, admit until date, and some personal details for my Fiancee. Also the sample on Immihelp website looks nothing like the one I have. Is the sample just outdated? Maybe CBP has changed procedures? Please advise.


2) Fiancee Petition approval notice is the NOA document that USCIS mailed out early in the Visa application process correct?

The recent I-94 you downloaded is now the correct and only I-94 format.
The petition approval Notice of Action is the second such notice you would have received from USCIS, when the petition was actually approved.
