Visa Status of old H1B and Next Steps

Hi,

My wife is travelling with me on H4. She has a approved petition from her old employer (Indian MNC). This petition was approved almost 5 years ago. I checked status of application in USICS site. It says "APPROVED" on may 2011 to be precise.

The questions that I have is.
1. Can she work in us with the above petition. What are the next steps that she needs to take?
2. What is the validity of the application?

Any help would be greatly appreciated.
