adjustment of status (I-485)

i have entered the united states in 1995 on a visit visa.in this coming july 2016 my daughter who were born here in the us will reach 21 years.so she will petition me for a greencard. but unfortunately i cant find my passport with visa , but i still have the i-94 card with me since it was kept seperately. i just want to know ,to apply for an adjustment of status will the i-94 card and a new passport be enough.
any help will be greatly appreciated. best regards.
