For I-130: arrived as tourist adjusted status to F-1. Which I-94 to use?

My husband is filing a petition in my behalf and as he was filling up I-130, he got stumped by Item #14.
I arrived in the US as tourist (B-2) then adjusted my status while in the country into student (F-1).
Which I-94 (number and expiration) should I use? And if I were to use my latest I-94 from F-1, should I still use my date of arrival as B-2?
