lost visa HELP!

Guys I need serious help. All my documents are ready to be filet for status adjustment but I am missing my original visa copy - the one I came to US with. My passport has expired in 2012 i believe, my consulate took an old passport and issued a new one, this time without a visa. Now I cannot obtain my original visa copy, no matter what. Any ideas what should I do, where to go and who to ask? Any advice is appreciated, many thanks
