question on Request for initial evidence - form i 864 - please HELP!!!

Hello, I filed my form i 485 & I 130 together on August. And recently I received the request for initial evidence from USCIS. The information they requested are:
1. Form i-864, Affidavit of Support, was submitted, however, page 3, part 4 is blank. (It is our mistake, we have filled out the info this time)
2. Form i-864, Affidavit of Support, was submitted from a joint sponsor; however, the names listed on page 1 and page 3 are not the same.
(The thing is we do not have any joint sponsor, my husband is the only sponsor for me (He has enough income to be a sponsor). And we only filed one form i-864, the petitioner is my husband.)

I am wondering what should I do to response this request? I tried to call USCIS but nothing helps. Do I have to submit another form from a joint sponsor? Or should I just ignore the second item, just send them the new form i-864 with completed information? My husband said it may because we leave the name on page 3 blank, therefore the system can not recognize whether we are submitted the form from a petitioner or join sponsor...therefore they request two items from us. I am really confused on it, and is there anyone else has similar problem before, or has some advice for us?

rfe for i 864

I received the exactly the same rfe today because we failed to put write my spouse name on page 3 of items 1a-1c. but have written the address. Still waiting here who had done it right and went on smoothly.. appreciate it much.

same RFE ON FORM I 864- LEFT page 3 part 4 blank

I received the same RFE and it says exactly the same. Two items because my USC spouse forgot to write his name on item 1a-1c of revision date 7/02/2015. First requirement is easy to comply. Just resubmit the completed Form I-864. I hope anyone with the same situation could share here, please. What did you? what happened? Was it approved? Really appreciate your response. Thanks!
