I-693, Report of Medical Examination and Vaccination Was not submitted(but i send it)

Hello. I submitted my I-485 with all the other form together including Medical Record (I-693) filled by the civil sergeant and they gave it to me in a seal enveloped but they also gave me the copy of my for so I can see what did they put in there.

Then i received a "Notice to Applicant" saying that Form i-693 has not been submitted or has been submitted and is not sufficient for the following reason(s):
- A complete and signed I-693 was not submitted.

Then they are saying that I should not mail it but bring it to the interview appointment.

So I am so confused. They saying that "A complete and signed I-693 was not submitted.", but then does it mean that maybe UNcompleted and UNsigned form was received? Not very clear.

I would be afraid bringing a copy of the form I send them because it just a copy of the original document. It mean i have to go to the civil sergeant, but they will fill it out the same way and does it mean I have to do all the test and vacancies and pay fees to the civil sergeant again?

There should be 2 signatures on the report. The top part you fill out either by computer or by hand, you must sign and print your name on the bottom of part 1. When you go for medical exam you should give your physician the complete package that you signed. The 2nd part of the report should be completed by your physician and he places it in a special sealed envelope. So either you forgot to sign part 1 or he failed to complete and sign part 2.
Here's what you should do.
Review your copy of the I-693, see if the signatures are there, if not correctly fill out your section. Next call your physician and make a follow up appointment and bring your copy of the I-693 along with USCIS RFE so he can review it and complete the 693 and seal it and bring it to your interview.

When did you file? At first when I read your post I thought it had something to do with the new requirement for the Syphilis test but it doesn't say so in the RFE you got so maybe it's not it.
If you sent it, maybe your doctor forgot to sign. If you have the copy and it shows that your doctor signed, it's unclear to me what happened but I'd go back to your doctor and asked for another Form I-693 in a sealed envelope just in case.

And we just called USCIS and they've told us information i can look up online in my account myself =/
So I guess I would have to go for another copy again, but does it mean I have to do all the tests again?

It sounds like the form was either not complete somehow, or wasn't signed (makes is still incomplete), or somehow USCIS messed up and lost or something.

You don't have to do the tests, unless any of the tests have condition such as "administered in the last x months" or so.

I would just go to the same doctor and have them check the form for accuracy, have them sign again with a current date. Bring the letter USCIS sent you.

Exactly, I would do exactly what Turk advised. Go back to your doctor, bring the letter USCIS sent you and ask them to check the form again. They have the results in your file, all they have to do is print it all over again and ask you and the doctor to sign again and seal it.

If it says "form I-693 not submitted or incomplete" then you'll have a complete one for the interview. If still not sufficient, then you can ask your IO face to face what is missing. I have no idea when you did your medicals but I went back to the panel physician to have a syphilis test done as I didn't have one done and I did it after CDC added syphilis as a communicable disease. If I get an RFE, new form I-693 will be ready for the interview.

Don't worry, go back to your doctor and get your new medical exam
