    My daughter in law is applying for adjustment of status in Portland Oregon. She had her medical examination in August before leaving China but no immunizations were given and she has no proof of past immunizations.
    Can she just get her immunizations separately at a clinic and attach this to her medical exam or does she need to have a full exam again?

    Was her physical ij China performed by a USCIS Civil surgeon?

    Proof of vaccinations. Most adults require proof of vaccination for Tet****-Diphtheria Td (within the last 10 years), Measles, Mumps, Rubella and Varicella (Chicken Pox). The Immigration Doctor will review your vaccination history with you to determine whether you have all the required vaccinations. The cost of additional vaccinations is not included in the price of the basic exam but they are offered through our office for an additional fee. Please bring all vaccination records to your scheduled appointment for the immigration physical.
    She may have to hace the exam performed again.

      Yes you can take your vaccines in another office

      Hi, yes she can take her vaccines in another Clinic. I think you need to understand the process so its clear for you.

      First, she has to find a certified USCIS Civil surgeon, you can just get into the USCIS website and you will find a list. Call and look for the best prices.

      They will ask her for an Immunization record, because she does not have one and she has not proof of which vaccines she has already taken before in China. She is going to have two options:

      * Take all the necessary vaccines again, or
      * Get blood tests to see which vaccines she already has taken.

      This two things can be done at the doctors appointment, now the Doctor charges a fee for the Medical Exam(sometimes includes blood tests) and a fee for each vaccine. It's not part of the Medical Exam to vaccinate her, but the Doctor has to see an Immunization report and check that she has all the necessary vaccines.

      This means that she can go to any other doctor and take the vaccines there, and then go to the USCIS surgeon and present him her new Immunization record.

      She will have to take the medical again, she cannot attached anything to it. The doctor will give her a copy of it and another one will go in a CLOSE ENVELOPE that only USCIS can open.

      Hope this helps, let me know if you have any other question. I had the same issue, as I did not have a Immunization report with me here and I was looking to not spend to much money.
