    Hi, all,

    I am trying to apply green card for my father. (concurrent filing)

    He entered U.S on 10/15/2016 by B2 visa, since he is in U.S now I will file the I-130 together with I-485.

    We live in Los Angeles, CA, and all forms for living(mailing) location I put LA,CA.

    But now since he is traveling in Chicago for a while, and since we need to file I-693 physical exam together with other documents,

    Below are my questions:

    Q1, can he do the physical exam in CHICAGO instead of LA? I can find an authorized doctor on USCIS website in Chicago so that he does not have to come back only for the exam. but I am worrying about the physical exam location is different than living location, will there be a problem?

    Q2, below is the document list I plan to submit, is there anything missing?

    I-130
    I-485
    I-864
    I-864A
    I-325A for me
    I-325A for my father
    recent pay stub for me and my husband
    employment verification letter
    bank statement
    passport photos for my father 4 each
    I-94 and passport and visa copy
    my naturalization certificate
    me and my husband's passport copy
    me and my husband marriage certificate
    me and my husband tax return for recent 3 years
    my birth certificate
    my father's birth certificate
    my father's divorce certificate(yes they divorced several years ago)
    check for I-130 $535
    check for I-485 $1225
    cover letter
    G-1145 wishing to receive e-notification

    file everything to IL:

    USCIS
    PO Box 805887
    Chicago, IL 60680-4120

    is the address correct?

    the medical location doesnt matter, as long as you have it, you're good to go

      1.No. As long it is a authorized doctor by USCIS.

      2. You Should also include I-765 (EAD So he can apply for social security and be able to work before the AOS process is over.) and I-131 (Advance Parole for traveling) there's no fee to file those, is good to have in hand just in case.

      4. Don't forget to sign, He needs to sign some forms.

          2. Thank you for remind, I thought both I need to file extra fee but just checked, they both free if filing with I 485.
          even we might not need to use it, they are good to have!
