Need clarification on affidavit that I am working on for "no name in birth certificate"
My birth certificate has no name of mine and my parents name is updated without their initials expanded. I am preparing an affidavit to be signed by my parents and mentioning the complete name of mine and so as my parents. now, here comes the question. when I was born, my second name was my father's. post marriage I got my second name changed as my husband's. my passport has my husband's name as the second name. so when I prepare the affidavit, what has to be given as my second name? should it be my father's name or my husband's name. can anyone please clarify.
