I'm almost ready to file all of my paperwork but I need some help from anyone who is/has been in the same situation.
I came to the USA in 2001 as a 10 year old girl. I came here on a B2 visa and I have all of the documents (old passport with the visa and I-94 slip). In 2013 I got DACA, I have graduated from college in 2016 and I am currently working. My work permit (through DACA) expires January 2018.
I married my husband whom I met in college April 2017. I already did my medical examination and now we are just putting all evidence and paperwork together.
We have copies of the marriage certificate, we have photos (trip to Las Vegas in November and the tickets, photos from the civil union ceremony, photo from August 2016 at his triathlon and all other photos) we have a joint account (will need to contact the bank about statement since we opened it the beginning of May), we have credit cards together, we have Verizon Wireless accounts together with both of our names, we will have same auto insurance but I first have to go the IL DMV to change my address on my DL (which was delayed since I did not have proper documents of my new address (bank statements). We currently live with his parents.
I know the basic documents and attachments that need to go with each form (passport photos and etc.) but any other advice, document that anybody can recommend, will be great. We are not getting a lawyer since we did a consultation and the lawyer herself told us that my case is straightforward.
Please, any help will be much appreciated.

Separate checks: should I do three (I-485, I-130 and biometrics or combine biometrics with I-485?)

Copy of husband's US passport
Copy of my old passport (one containing the Visa), form I-94 and my new passport (biographical page from both passports)
Copy of Marriage Certificate
Passport photos (2) of husband
Passport photos (2) of myself w/A#
Evidence: bank statement, credit card statements, flight ticket to LV, Verizon Bill, photos of us two and with other family members
I-485
Copy of old and new passport's biographical page
Copy of old passport's page with Visa along with I-94 form
Copy of Marriage certificate
Copy of Polish Birth Certificate with Translation
Passport photos (2) of myself w/A#
I-693 form sealed in an envelope provided by the doctor
I-864 with husband's taxes (I am pretty sure he knows but how far back?) I also did my taxes so should I included that somewhere?

Also we are filling out the forms using my maiden name since I haven't had the chance to do that with SS and everywhere else. What I have read on some of these threads is that you can notify the person doing the interview about your name change at the interview? Is that correct?
