Missing information in I-693

Hi,

I just noticed that on my I-693, i had not filled the identification information(Name and A#) in 3 pages out of the 13 pages(I noticed in my copy that i have). Would this lead to denial? I had already received an RFE for this form for the surgeon missing to notate certain results. Now, when I responded to it with all of the required information, this identification information was missed as it was handwritten in the doctor's office(We happened to notice that the form version I typed and printed had changed and there was a new version). What can I do now? Can I have the surgeon fill it once again and send it in a sealed envelope so that it reaches the adjudicator before he makes a decision?

These things usually take forever to make a final decision on. If I were you and have enough money, I would go to the doctor to make the correction and send it overnight. Missing A# is a big deal. Since you've already had a previous RFE.
