In part 1 for answer to question : When I last arrived in the United States do I check Box: 22a: inspected at port of entry and admitted as: then type in box " fiancee visa" (it won't let me type K-1 visa; OR check box: 22d: "Other" and type in that box "K-1 visa"?

Yes, you were inspected. Either "K-1" or "K-1 fiance" will be fine.
