Information Requested on The Form at Biometrics Appointment

Hi -
Need help from I-485 applicants who had their biometrics appointment in the last 30 days.

To be ready for my Biometrics Appointment I want to know what information you had to put in Biometrics Form you had to fill out the day of the Biometrics Appointment. Thank you.

My husband had to fill out basic information like: height, weight, hair color, eye color, etc.

Particulars

They ask for your eyes color,height,hair color,weight,citizenship,full name,by the way if someone goes along with you they don’t need to bring purses or cellphones,it takes about 5 minutes the whole process,they will give you a survey about the Costumer service
