Please advise on which forms to file together for concurrent filing. Is the medical part of this package?

I would send the medical AOS rarely takes a full year ... but you dont have to send it then you will get a letter a few weeks in that says: please bring medical to interview..

I-765 for work permit
I-131 for travel permit

You can send it with the I-485 or send it later in response to an RFE. Sending it later is mainly useful only in categories where AOS take a really long time, or where the category dates might retrogress, so the medical might expire (it expires after 1 year), but nowadays many categories are slow, so it's hard to say.
