Hello all,

Form G-1145 - e-notification of Application/Petition Acceptance

we are filling I-130 and I-485 together,

we are aware that for I-130 , I should put my(Petitioner) contact information.

But, I-485, I-765 and I-131- these forms are filed by Beneficiary, so should we put Beneficiary's Contact information on G-1145 or petitioner's? Do I have to attach separate G-1145 forms for each form?

You need a separate G-1145 for each application if you want to receive updates. We filed AOS for my wife and kids and on each G-1145 i put my contact information. There was no issue, i got an email and txt when applications were revived etc
So go ahead and put whoever needs to receive the notifications

You only need to fill out one and place it in the package. I’ve seen examples on this site where the petitioner filled out one and the beneficiary filled out the other 3, but the petitioner still got all 4 notifications. Personally, we filled out one 1 form and got all 4 notifications.
