I-765 - I already have a Social security card

Hi, on an ESTA, got married, applying for a green card.

on I-765 question 9a. says "have you ever been issued a social security card?"

Yes I have, while on a j1, however on my card it says "valid for work only with DHS authorization"

Question 10. says "Do you want SSA to issue you a Social Security Card?" Should I answer yes as my current one has a limitation on it?

You have to answer No in question No 10, since you have one, the same thing happen to me , I have one that say "not valid for employment", the I changed it for the one that you have, until we become legal resident and we can take that restriction from de SS

item 15 ask if I have ever applied for employment authorization from USCIS.

I have been on two J1 visas where I had permission to work. Does that mean I or a sponsor on my behalf applied to USCIS for employment authorization before?
