EAD/AP Renewal - Please help

Please help, my EAD expires May 4, and I need to renew it to be able to continue to work. I don;t have much time to mess around with so I need to make sure I get it right the first time that I send the paperwork. Please anyone that has renewed AP/EAD before can you please tell me what you sent? and if what I have below is ok?

From what I have read online, I need to send almost the exact same information I sent with my Adjustment of Status Packet:

*G-1145
*I-765 filled out -- if my last re-entry was with the ComboCard, what should I write as Status of Last Entry? Would it be I-485 Pending? For #15, how can I find out which USCIS office I applied to for my last EAD?
*Scanned passport (includes expired H1B visa)
*EAD front and back
*Two identical passport sized photos (Name written behind)
*I-94
*Filing Fee - Check for $410 .. do I need to pay for biometrics fee again ($85)? Do I need to send a check for $495 instead of $410?
*I-131 filled out-- if my last re-entry to the USA was with the ComboCard, what should I write as Class Admission? Would it be I-485 Pending?
*Second copy of Scanned passport
*Second copy of I-94
*Separate sheet of paper, explaining how I qualify for an Advance Parole

Then based on https://www.uscis.gov/i-765-addresses I need to mail it to:
