I 485 processing time

I applied for I 140 in NBC in Jan 2018. After NOID my I 140 in EB1b was approved on 25th April. I cannot apply for I 485 since it is not current for Indian citizen. My spouse and I are both from India. I wanted to know how long will it take once we file our I 485 in october for us to get green card?
Should we send our medical at the time of filing I-485?
I may not have job by May 2020 so I am worried if my green card will come before that or not
