In that case, you can furnish a notarized Affidavit of Residence. I would also get an advance health directive and power of attorney. And of course, I would mimic the format on the Table of Contents put together by Emerald 2020. Full sententces etc.

Yup, generate it, print it and then, get it notarized. I would call the UPS branch first to make sure the notary will be there. Otherwise, there is always one present at banking institutions.
