  • I-693: Submission of Medical Records to Civil Surgeon

    Hi all --

I've got my physical exam for the I-693, soon. Today, I received an email that I have to bring my medical records for all conditions I'm currently being treated for. Did you have to do the same?

I've been treated for depression (no suicidal thoughts etc.) and that record is quite long. It has a lot of detail my therapist put in. Do I have to bring this 8-year history that has descriptions of family/work problems? Or can a medical record also be the list of appointments and the diagnosis? I'm really uncomfortable that a physician might be reading all of that.

The physician also wants me to bring prescription glasses and my medical records for a chronic disease (that doesn't fit into any of the categories that are relevant for admission).

I'm starting this thread because I'd like to get a better understanding of what I have to bring in legally and what information these documents must have.

Thank you so much for reading my post!

Since it sounds like you've already disclosed your previous depression and chronic disease to the civil surgeon, then most likely you will need to disclose your records to him.

My Experience: Other than colds and flu, I've never been treated for any health conditions. The only items I brought to the medical exam was my vaccination records and ID.

Can you please share your experience of what happened? I am in a similar situation as well.
