What happenes if an I-485 applicant cannot provide documentation of having received vaccination against vaccine-preventable diseases? Can a USCIS designated civil surgeon give those tests with the required medical examination and include them in the Form I-693?

Yes, if fact even you had documentary proof you would have to show it to the civil surgeon so that he can certify that youre properly vaccinated on form I-693.you couldnt just send it on your own
