Affidavit of Support

I have applied for I-140 in NIW category and received the receipt notice. I am now planning to file I-485 both for me (H1B) and my wife (H4). We both are in the U.S.A. I would really appreciate it, if somebody could confirm me, whether I need to include an affidavit of support for my wife's I-485. If yes, what form should I use: I-864 or I-134?

I am not sure on this one, but you should USCIS customer service and they will give you this information.
