List of forms and questions, plz help!!

I'm adjusting my status. I'm currently holding H1B visa and just got married to a US citizen. Here is a list of forms I have so far:
1. I-485
2. I-130
3. I-765 (work authroization)
4. G-365A (one for me, one for my husband)
5. I-864 (Affidavit of Support)

1st Question: Do I miss anything? Also, Do I need to file I-693 (Medical Exam) as well? or can I do that later?

2nd Question: Do I receive separate receipt for each form or just one receipt for all?

3rd Question: I need my I-765 in order for me to switch to a new jobs. However, I heard that since I have H1B, filing I-765 can allow me to switch jobs as long as I file it before my start date. Is that true that I don't have to wait until I receive the actual employment authotization card?

4th Question: I don't have my birth certificate, can I just show them my passport, which indicates my bithdate as well as the birthplace?

4. I think you each need 2 copies of the G-325
Ita always best to sned everything but you can send the medical later or take itw ith you to the interview. Depeneding on you rluck too, if you wait a yr or mor efor the interview, the medical wont be vaild if you send it now.

They may request your birth certificate. Send the copy of th epassport now but they might write to you late rto request the BC
