    Hi Cicil,
Yes now you need to get an attorney and it has to be someone very good in immigration cases. Will not be easy, but don't give up.I got my letter about intend to denied 3 monts ago,I went to good attorney and I got approval for my GC 5 weeks later. So don't waste your time and start working on it.This is your last chance.I don't know what 8 CFR Section 103.5 mean.

Does it really matter now that you were not sent a letter of intent to deny? You now have a letter of denial and thats what you need to address.
I assume your case was denied because you didnt complete the medical requiremnets I.e. the vaccination requirement. The notice asking for it ( the second time according to you)has at the bottom a note which clearly says that if you do not send the info within 12 weeks your case will be denied. What further notice did you need?

A Motion to Reopen is simply a request asking USCIS to reopen the case and stating why.
I am not sure if you were saying that you sent the vaccination form and they say you didnt or you're saying that you didn't send it. If you in fact sent it and they deny receiving it, then use the form enclosed in the denial letter to do the Motion to Reopen. Send the evidence that you have to show that (1) you did meet the vaccination requiremnets and (2) you in fact sent it to USCIS within the timeline. The fee I think for the motion to reopen is $335 or soemthing like that and I think you have 30 days after the date of the denial letter in which to do it.

However, if you in fact never sent it, it might be in your interest to hire an attorney to write the motion. In that case, you would have to get vaccinated either by a designated medical doctore or get vaccinatd elsewhere but have the form signed by a designated civil surgeon. You attorney would have to provide a compelling reason for why the info wasnt sent on time.
