Can somebody help me how to file the form. I just got married i have 2 names river jane and my middle name is howard my last/family name is smith then i got married to scott. what would be my family name and my middle name? Correct me if im wrong...... am i going to used the smith as a middle name and scott for my last/family name?

Its your choice as to which name you use.your maiden or maried name....you can have your doc nad GC in your maiden name if you choose.............youre married, regardless of what name you chose to useOriginally posted by crazyfrog1921bComment
