I am currently filling out the various forms to adjust my status. And i have also changed my last name to my husband's. What should i write on the form as my last name. Should i hypernate my name then his or should i just write his last name. Also on the I-485and part 3 and #12 should i tick yes or no as i came to the States on a J1 visa and was subject to the 2 year home residency rule, i applied for a waiver and got it off what should i tick at #12. Also on the same form at Part 2 what should my answer be. ( I am married to a united states citizen and i also got approval from USCIS of my waiver to go ahead and adjust my status. Someone please help me. Thank you
