Birth certificate

My passport has my mom's first name only

While writing the affidavit to support non-availability of my birth certificate, should I have to mention my first name and last name? If I do that it is going to clash with my passport data.

Please advice what needs to be done in this case. Answer to this question will help multiple people that are working with me. Thanks in advance
