H4 document for filing I-485 - help please

I need to provide H4 document for my wife while applying for I-485. She did not had any H4 document. She had H4 visa stamped in her passport before coming to US. Can someone please advice me what should I provide? Is the H4 visa page itself enough?

I -94 as well as the copy of visa stamp

She does not have that I-94 now as she changed to H1 and went to India after that couple of times. just the visa stamping would be enough?
