I-485 processing information

When entered US last time as H1B worker with valid INS approval, my cousin didn't have H1B visa stamp because she had Canadian GC. In view of the above, how to answer questions like nonimmigrant visa#, where visa issued or date visa issued in filling out Form I-485 page 2 part 3(processing information).

I would appreciate so much if someone can help with advice or share experience of similar situation.
