Which passports to copy?

I have 2 passports - one that is expired and contains my entry stamp when I first entered the US and one that is current and has not been used yet (I renewed when the other ran out while I here in the US). Will I need to send copies of both??

And just so I'm getting this right, I'm under the impression that I need to copy:

front and back covers of passport
all pages that have port of entry stamps
biographical page with photo

Is that correct?
