Marriage Certificate and Immunization Record for I-485

I am 47 yr old Indian citizen;I do not have a marriage certificate (we were married in India). Also, both I and my wife do not have any WRITTEN immunization record, we do have immunization record of our son (12 yrs old).

1. Any information about the FORMAT of marriage certificate for I-485 application would be appreciated?

2. Any suggestions regarding immunization requirements for us and how to fulfill it would be appreciated.

1. When you go to civil court in India, any typist or lawyer who will do this for you will have the format of the marriage certificate or affidavit. You should not have any problem.
When you get it like that, please send us the format and we will publish it here for everyone's benefit.

2. You just go to INS approved doctor in US. They will give you all/any necessary immunizations that you would need and would be appropriate for your age.
