I130 is approved for parents, Should I file for 485 if they are in US now???

Hello,
My parents’ I- 130 got approved few days back. When I filed their GC, they were in India. They came here in Jan 2009 and now I am thinking of AOS (adjustment of status). Is it a good idea to do that, or it will create confusion, as when I originally filed, I mentioned they are outside the country.
If I file 485 now, what other documents I need to send along?
Should I send medical reports at the same time, or it will be required in later process. please advice what should be the best thing to do?
