Approved H1b but no pay stubs currently on h4

Firstly H1b approved on Oct 2008 with employer A.Secondly my H4 approved with my spouse employer.No pay stubs on h1b.Never worked.
My current status is h4.
1.How to change my H4 to H1b?
2. Is that fine to change my status from h4 to h1b any time during the 6 years of period?
3.what if i can not able to find a job continuously on h1b will my h1b get affect?
4.If whenever i can not able to find a job continuously my status automatically goes back to h4 or do i need to file for H1b to h4?
5.What are all the documents needed to apply for cos H1b to H4?
6.Never worked on H1b no pay stubs .... Is that possible to apply for H4 to H1b with out any paystubs?
7.How much the change of status cost?
