I- 485, Copy of passport page with nonimmigrant visa

1. I - 485 is saying " If you have obtained a non immigrant visa from a U.S. embassy or consulate abroad within last year, submit a photo copies of the of your passport containing the visa."

this year I was on H1b visa till Mar 09 thn I apply for change of status to F1 in April while without leaving USA. I got F1 approved in may but as it was COS within US, I did not leave country and I get my F 1 VISA. In this case wht should I do?

2. I 485 need evidence of status. I get F1 approval receipt and approved I 20 from Uscis. Can I show it as evidence of status. I am also thinking to get a letter from my Uni. saying that I am current F1 student studying at the UNI.

Please guide me in this matter.

I think you need to make copy of your passport (don't need to make copies of blank pages), attach copies of your COS receipts/us-visa-and-immigration-forms/letters and on a cover sheet explain the situation.
