Question about filing I-485

Hi,

I am little confused in filing I-485, EAD and Travel document:

1. With I- 485, filing I-131 and I-765 concurrently, do we need 6 photographs ( 2 with each form) or total 2 photo needed.
2. While OPT A#_______ is allotted. While filling now I-765 (EAD), do we refer OPT A#___ or ignore? Is this first time filing or we consider OPT as one EAD and this is second time filing? What to enter in I-765?
3. Where can I find non immigrant visa number asked in I-485.
4. What I need to put in I-131, advance parole or reentry permit?

My case is a self petition E11, though I attached a reference letter from current employer stating 'we intend to continue "employee" on board with "employer firm" on approval of permanent residence' which is now 3.5 months old.
1. Do I still need an emplyment letter?
2. Can the old letter without stating $xxxxx amount work?

This forum is extremely helpful for intending immigrants. I am using this for last 5 years.
