I just noticed that the USCIS expedite criteria has been simplified. Please see https://www.uscis.gov/us-visa-and-im...pedite-request

In our case, we filed under expedite criteria that is no longer listed ("Nonprofit organization whose request is in furtherance of the cultural and social interests of the United States"). My husband was invited to speak at an overseas conference so we submitted letters from the conference host and the US university that my husband is affiliated with. In your case you will need other kind of evidence -- letter from your wife's doctor detailing her condition and inability to work, as well as financial and/or tax documents showing the severe financial hardship if you're unable to work. Think like a lawyer, build a case, and then submit it to your congressperson or senator for then to forward to USCIS on your behalf. Good luck!

How long did you all have to wait after receiving the following message: "Thank you for your inquiry regarding this constituent’s immigration matter. This case will be routed for expedited processing. Once this case is reviewed, your constituent will receive a decision or notice of further action. Due to the extremely high volume of expedite requests for this case type, there are numerous cases involving exigent or humanitarian issues already in the expedite queue, and we are unable to provide a specific timeframe for the expedited review. The strict enforcement of the expedite criteria means that all cases sent for expeditious handling are urgent, and we assure you that this case will be adjudicated as soon as possible."?

If you have recently requested an expedited processing of your case/card, have you been getting any updates via email/case status sites? I read that - "To increase efficiency in the review and processing of expedite requests, USCIS is not required to provide justification and is not required to respond regarding decisions on expedite requests." My assumption is that this is new and I am wondering whether any recent requests have been getting updates on the progress.
