  • USCIS - LOST my I-693 form!

    Hi All,

    Need immediate help! Both my wife and I received a courtesy letter from USCIS yesterday stating the following:

    "An initial review of evidence submitted with your application to register permanent residence or adjust status determined that form I-693, report of medical examination and vaccination record, has not been submitted or has been submitted and is not sufficient for the following reason(s):

    A completed and signed I-693 was not submitted.

    You will be required to submit information and evidence to address these deficiencies."

    I had already submitted an unexpired form I-693 in a sealed envelope along with my AOS application in February 2019. Not sure why they find it missing in our application. I'm not quite sure if i should just bring the copy of the original form (unsealed), or should I go back to the civil surgeon and ask for a replacement copy in a sealed envelope ($250/person).

    This is so frustrating, cos I have no idea what the USCIS need at this point. If anyone has knowledge on this, please enlighten me..

    Sucks, but it's better to get a new one, unfortunately. You could also wait and see if your 693 somehow turns up during interview, but I highly doubt it. The cost of a new medical very much worth it.

      That's sucks. Another $500 down the drain.I would probably request for a replacement copy in a sealed envelope. Hopefully the civil surgeon would provide it with no additional costs. Wish me luck!

        some civil surgeon would be happy to do so for free / minimal cost. Mine charged $200 even though no new test was done.

          Mine is $250/person.

          Reconstruction of the Form I-693 sealed envelope is $250 per person.

          But as hpx3000 said that it will be worth it.

            I have the same problem... my interview is also on 11. When you reconstructed your form did you use the new or old one ? Did you update the dates or kept the original dates ?

              Do you have the copy of the forms submitted. Did you make sure to sign where needed?
