I didn't apply Employment Authorization (EAD), Now I'm going to apply it. My question is

WHAT DOCUMENTS SHOULD I SEND WITH THE FORM (I-765)?

What is the fee?

Send with a copy of I-797 you get in the mail for successful I-485 application. No fee is needed. Recommended document list can be available on this site, just search.
