Last week I finished the I-693 phase.
We did not submit the form above with our 485. We just waited for the courtesy letter about the physical exam.
I recommend the physical exam when you get the letter with the day of your interview.
My wife a book of all the shots taken in her country and it saved me hundred of dollars.
Her physical exam, blood and urine test were done in Wisconsin and I attached the prices charged by the hospital. (with check mark). I am sure I could find better prices but I had not too much time for this.
Also, glad that everything was fine. Otherwise, It would cost way more. A simple X ray is expensive (in fact everything else).
