Advice needed on i-485 form

I am a USC and is preparing paperwork for my mother's AOS. My father passed away ten years ago, so I check the current marital status of my mother as 'Widowed' in question 1 and type 1 time in 3 under Part 5 (page 7/18). My question is whether or not I should fill out my father's information as prior marriage of my mother for question 11-16 with his death date being considered as date marriage legally ended. Or should I leave question 11 thru 16 blank?
Also, will I be asked to submit a certificate of death of my father (which I haven't found out yet) if I fill out his information in question 11 thru16.

Just put DECEASED... that's what I did and did not include death cert or anything like that.
