I understand that I need to submit all my prior visa approval notices (I797) to prove that all my stay in US had been legal. I miss one of these notices. Is there a way to get a duplicate copy from INS? Please advise.

Your I-94 your visa stamp in the passport should be OK. Somewhere if you have a copy of the old visa is also OK. Also, your 1040 tax forms will prove that your stay was authentic. If you have your work permit that also works
